The Company has established the following procedures for responding to a request for disclosure, etc.
1. Contact for a "request for disclosure, etc."
- For "disclosure," "change," or "deletion (withdrawal from membership)" pertaining to online members, customers can perform these actions by themselves through Mandom's website.
- For a "request for disclosure, etc." of other personal information, such as written applications or questionnaires, please contact the help desk for personal information.
2. Method of identity verification upon a "request for disclosure, etc."
- For online members, their identity will be verified by their ID and password.
- For other personal information, such as written applications or questionnaires, the person making the request may be asked to provide his/her identification card or other documents to verify that he/she is the individual concerned (such as a driver's license, health insurance card, or student ID card).
3. "Request for disclosure, etc." by a representative
In the case of a legal representative
If the person making a "request for disclosure, etc." is the legal representative of the individual concerned, a minor, or an adult ward, or a representative entrusted by the individual concerned to make a request for disclosure, etc., please provide the documents listed below.
- For online members, their identity will be verified by their ID and password.
- For other personal information, such as written applications or questionnaires, the person making the request may be asked to provide his/her identification card or other documents to verify that he/she is the representative (such as a driver's license, health insurance card, or student ID card).
In the case of a privately appointed agent
If the person making a "request for disclosure, etc." is the legal representative of the individual concerned, a minor, or an adult ward, or a representative entrusted by the individual concerned to make a request for disclosure, etc., please provide the documents listed below.
- One copy of a power of attorney
- One copy of a seal registration certificate of the individual concerned
4. Fees for a "request for disclosure, etc." and how to collect them
The Company will not charge administrative fees for disclosure procedures, but customers need to bear the costs for obtaining the necessary certification documents to be submitted to the Company, as well as postage and communication costs and other expenses.
5. Handling of the provided certificates, etc.
The provided personal information will be used to verify the identity of the individual concerned or his/her representative, and will be stored for one year after the completion of the response to the request for disclosure, etc., after which it will be disposed of.
6. Reasons for nondisclosure
In the event of nondisclosure for any of the following reasons, the Company will notify customers to that effect.
- When the identity of the individual concerned or his/her legal representative cannot be verified
- When there is risk of harm to the life, body, or property of the individual concerned or a third party
- When there is risk of significant interference with the proper execution of the Company's business
- When laws and regulations are violated
